Enclomiphene

Enclomiphene is a non-steroidal selective estrogen receptor modulator (SERM) primarily used to treat male hypogonadism, a condition characterized by low testosterone levels. As an isomer of clomiphene, it offers targeted hormonal modulation by stimulating natural testosterone production without compromising fertility. Enclomiphene has emerged as an effective alternative to traditional testosterone replacement therapies, providing a safer and more physiologically balanced approach to managing hormone deficiencies.

Medical Uses

Enclomiphene is primarily prescribed for the treatment of secondary hypogonadism in men, which is often characterized by reduced testosterone production caused by dysfunction in the hypothalamic-pituitary-gonadal axis. Unlike direct testosterone supplementation, enclomiphene helps restore natural testosterone production while preserving fertility, making it a preferred option for men desiring to maintain or improve reproductive potential. It has also been explored for off-label uses, such as improving muscle strength, energy levels, and overall vitality in men with low testosterone.

Mechanism of Action

Enclomiphene works by selectively blocking estrogen receptors in the hypothalamus, preventing estrogen’s inhibitory feedback on the hypothalamic-pituitary-gonadal axis. This inhibition leads to an increase in the release of gonadotropins—luteinizing hormone (LH) and follicle-stimulating hormone (FSH)—from the pituitary gland. Elevated levels of LH and FSH stimulate the testes to produce more testosterone and sperm, thus addressing the underlying cause of secondary hypogonadism.

Side Effects

While generally well-tolerated, enclomiphene may cause some side effects. Commonly reported adverse effects include headaches, nausea, hot flashes, and mood swings. Rarely, it may lead to visual disturbances, dizziness, or more severe complications such as deep vein thrombosis (DVT). Patients with pre-existing health conditions or those prone to clotting disorders should consult their healthcare provider to assess their suitability for enclomiphene therapy.

Interactions
